Adults only — ketoconazole
Daktarin Gold is licensed for adults; children need a pharmacist's advice on an alternative. Keep away from the eyes, and stop if the skin becomes red, itchy or sore in a new way. Ask a pharmacist first in pregnancy or breastfeeding.
Warfarin and other blood thinners
Miconazole and ketoconazole can increase the effect of warfarin and other vitamin K antagonists, even applied to the skin. Tell the pharmacist if you take a blood thinner; you may need an extra INR check.
Keep going after it looks better
Fungal infections come back if treatment stops early: continue for at least a week after the skin looks clear (ten days for nail-fold infections). Keep away from the eyes. It can weaken latex condoms and diaphragms. See a pharmacist or GP if there's no improvement after four weeks, or if the scalp or nails themselves are involved.

Why ketoconazole

Ketoconazole is one of the most potent topical antifungals, which is why it's the choice for athlete's foot that's come back after other creams, or the thick, scaly moccasin-type infection on the sole that milder treatments never quite shift. It's applied once or twice a day rather than several, and an itch that's been maddening usually settles within days.

How to use

Adults: wash and dry the area, then apply once or twice a day to the rash and a margin of skin around it. Athlete's foot: four to six weeks. Jock itch or sweat rash: two to four weeks. Keep going until the course is done even if it looks clear sooner.
