How it works
Orlistat isn't a stimulant or an appetite suppressant; it stops the fat-digesting enzymes doing their job, so the blocked fat passes out of the body instead. That's why it only works with a diet that keeps each meal to around 15g of fat: too much fat in a meal and the treatment effects (oily stools, wind, urgency) make themselves known.
How to take
One capsule with each of three main meals a day, during the meal or up to an hour after; skip the dose if you skip a meal or it contains no fat. Never more than three a day. Take a multivitamin at bedtime, because orlistat also reduces absorption of vitamins A, D, E and K. If you haven't lost weight after 12 weeks, stop and talk to your GP; don't use it for more than six months without medical advice.
Good to know
Not suitable in pregnancy or breastfeeding, with ciclosporin or warfarin, or with a malabsorption condition. Tell the pharmacist if you take medicines for diabetes, blood pressure, cholesterol, thyroid or epilepsy, or an oral contraceptive: severe diarrhoea can make the pill less reliable, so use extra contraception if it happens.
